FV Cage Equipment Reservation Form

The FV Cage Equipment Reservation Form should be completed at least one (1) week prior to any type of production. Your request can be made for any date in the current semester. 

FV Cage equipment pickups and drop-offs will be scheduled by appointment only on Mondays-Fridays (except Holidays) during business hours.

Completing this form does NOT guarantee the equipment you are requesting is available.

*You receive a Unique ID number when you submit an FV Project Form. If you have already completed an FV Project Form, you can find the Unique ID number in the confirmation email you received after submitting the form. Will you be shooting inside a motor vehicle in your production?*
Please mark “yes” if you plan to shoot a motor vehicle that may or may not be moving. If you’re simply using a car to drive to location, you can check “No.”

Will you be using smoke or fog?*
For example, you may wish to use a haze machine for your production.

In addition to this Student Project Application, you must fill out the Out of Country Insurance Form for any equipment that is being taken out of the US.
